Our care homes are registered with CQC and commissioned by NHS and local authority partners for people whose needs require more than standard community care can offer. Our Community Housing offers flexible accommodation to meet peoples needs and aspirations.

LEARNING DISABILITY SUPPORT

Long-term residential and supported living for adults with learning disabilities

Our learning disability homes are our most established services, shaped over years into environments where people genuinely belong. We support adults across the full range of learning disability presentations, including those with associated autism, epilepsy, sensory needs, and complex physical health conditions. Our homes are both professional and family orientated, using person-centred, active support approaches that put real outcomes at the centre within a supported home environment.

SPECIALIST MENTAL HEALTH SERVICES

Complex, high-acuity mental health provision

Residential and Community-based residential care for adults with severe and enduring mental illness

Our mental health service is designed for people who sit at the intersection of high clinical need and complex social circumstance, individuals for whom standard CMHT or generic supported living has not been adequate. We accept referrals that others decline, working with presentations including treatment-resistant psychosis, personality disorder, dual diagnosis, and significant forensic histories, within a structured therapeutic environment that prioritises safety, dignity, and recovery.

From crisis to possibility: the impact of specialist mental health support

Many of the people who come to us have spent years, sometimes decades caught in a cycle that the system struggled to break. Repeated hospital admissions. Periods of acute crisis followed by discharge into circumstances that couldn't hold them. Medications adjusted, professionals changed, placements that broke down. For many, this had become the shape of their life, and hope had quietly disappeared from it.

By the time they reach us, some have stopped expecting things to be different. They have been told or have come to believe that stability is not something available to them. Aspirations, relationships, a sense of purpose: these can feel like things that belong to other people.

What we see, consistently, is that this is not the truth about these individuals. It is the truth about what they had previously been offered.

In our services, people who had experienced nothing, but relapse begin to stabilise, not because their diagnosis changes, but because the environment around them does.

Consistent, trusted relationships with staff who understand their history and don't give up on them. Structures that provide safety without removing dignity. Support that is responsive to the person, not just the presentation.

The changes we witness are not small. People who had not left a building without incident in years begin to access their communities. Those who had lost contact with family begin to rebuild those relationships. Some begin to speak, sometimes for the first time with any real conviction, about what they want their future to look like.

This is what specialist, relationship-based mental health support can do, not for everyone immediately, and not without difficulty along the way, but reliably, for people that other services had found it hardest to reach.

REABLEMENT SERVICES

Structured recovery towards independence

Short to medium-term support following hospital discharge or significant change in need

Our reablement services offer structured, goal-directed support for adults who have experienced a significant change in their functional capacity, whether following an acute hospital admission, an exacerbation of mental or physical ill health, or a breakdown in an existing care package.

The focus is on restoring skills and confidence progressively, with clear milestones and a defined pathway towards greater independence or a planned longer-term placement.

What we provide:

•  Post-hospital discharge and step-down support

•  Functional skills and daily living retraining

•  Crisis recovery and stabilisation

•  Medication management and health monitoring

•  Goal-based support planning with clear outcomes

•  Transition coordination with community and statutory services

COMPLEXITY RANGE

Time-limited support focused on measurable recovery — from those needing a short period of reablement through to individuals with significant physical and mental health comorbidities.

All services are registered with the Care Quality Commission (CQC) and commissioned by NHS integrated care boards, local authorities, and specialist commissioning teams. We work closely with multidisciplinary teams, clinicians, and families throughout the referral process. Our Community Housing offers flexible accommodation to meet peoples needs and aspirations.
